The Sonneborn Hearteam Fund was established by the Sonneborn family, together with a group of close friends, to support the Cardiac Rehabilitation program through scholarships and the purchase of essential equipment.
Over the years, this fund has become an invaluable resource for Cardiac Rehab—now known as Health Hearts Active Lives. Contributions have helped purchase critical supplies, including blood pressure cuffs, Kardia Mobile heart-monitoring equipment, and other tools necessary to deliver high-quality, effective programming.
Even more importantly, the fund has helped remove financial barriers for seniors by providing assistance with memberships, allowing them to participate in this life‑saving program at no cost. This support ensures that individuals can focus on improving their heart health, building strength, and maintaining an active lifestyle—regardless of their financial situation.
